Treatment Foster Care Case Manager
About your role:
Our Treatment Foster Care program offers a family-based placement option for children with serious behavioral, emotional, or medical health needs, who require more intensive interventions and supports than traditional foster care is able to deliver. You will work with foster parents, Department of Health and Welfare caseworkers, clinicians and birth parents to develop individual treatment goals for the child/youth while they are in the TFC program. This position coordinates the delivery of behavioral health and other community-based services in order to support the child/youth's service plan, maintain a stable placement in the home, and work with the child's own parents towards permanency. Our Case manager acts as a pivotal member of the TFC Treatment Team, linking the foster child to necessary services (academic, physical, mental health, behavioral health, etc) monitoring each youth's treatment goal needs, progress, and helping to determine when they are ready for discharge. Our case managers are provided the support of a team environment, ongoing training, and clinical supervision, for their professional development and to most effectively aid in their clients' success.

What we are looking for:
  • Bachelor's degree: Human Service field a plus (Social Work, Psychology, Sociology, Counseling, Family Relations, Nursing, Physical Therapy, Occupational Therapy, Therapeutic Recreation, Speech Language Pathology, Behavioral Sciences, Art Therapy, etc)
  • Ability to pass background checks, including Enhanced Fingerprinting with ID DHW.
  • Valid driver's license with reliable transportation and proof of car insurance
  • Experience utilizing electronic health records system a plus
